TruHealth Alliance serves as the lead applicant and coordinating entity for integrated rural health initiatives. We are responsible for program design, partner alignment, grant execution, fiscal management, and outcomes reporting.

We don't just aggregate. We lead, design, and deliver.

Our Approach

How We Build Rural Health Systems

Care Coordination

We coordinate primary care, behavioral health, peer recovery, and community health workers into unified care teams. Our network supports post-discharge follow-up, reduces avoidable ED utilization, and connects patients across the full care continuum.

Technology Integration

We deploy population health technology supporting admission and discharge notifications, telehealth delivery, remote patient monitoring, Social Determinants of Health (SDOH) screening, and intelligent, automated care coordination workflows across consortium partners.

Workforce Development

We build rural health workforce pipelines by training community health workers, peer recovery specialists, certified nursing assistants, and allied health professionals through partnerships with community colleges and workforce networks.

Value-Based Care

We design geographically defined clinically integrated networks within Maryland's AHEAD model, aligning providers under shared governance and shared savings to improve outcomes and reduce total cost of care.
